How to add/remove care plan conditions?

How to add/remove care plan conditions?

  1. Open the patient record in Inca
  2. Navigate to the Care Plan tab
  3. Select Rebuild Care PlanScreen_Shot_2019-07-26_at_4.17.58_pm.png

 

  1. Select Modify current care plan and NextScreen_Shot_2019-07-26_at_4.20.08_pm.png

 

  1. Select/deselect any predefined conditions and click Next. You can also Add Custom Conditions where necessaryScreen_Shot_2019-07-26_at_4.22.44_pm.pngScreen_Shot_2019-07-26_at_4.44.21_pm.png
  2. Select action as appropriate then Modify. You can choose to add to the existing plan, start the plan from scratch or keep the existing plan.

   7. Where the care plan has already been approved, Select Accept Changes at the top of the Care Plan tab. Alternatively, Approve the care plan once you have finished editing
